All customers on COD terms or clients paying with a credit card, are required to fill out a Credit Authorization Fom to be kept on file. We will gladly accept, Visa, Master Card and Amex
- Please fill out the Credit Card Authorization form in its entirety (fill it out on your PC or Manually)
- Sign and date where requested
- Fax back to our office (Delaying this process may result in order processing interuption
